Why does the blinking led example in AN-951 not work?

I am trying to get some custom hardware up and running and the blinking led example does not work.

I can manually toggle the output pins by using the read/write registers tool but I cannot generate the square wave on the output pins with the example schematic. I have tried on this on the EVM board as well and it does not work there either.

Anyone have some experience with this example program?
